About The Position
Phoenix Home Care and Hospice is looking for a highly organized, solution-focused Care Coordinator to help ensure our clients receive the exceptional care they deserve. This role serves as the connection between our clients, families, caregivers, and nurses, helping ensure the right care is delivered at the right time. The ideal candidate thrives in a fast-paced environment, enjoys building relationships, solving problems, and keeping multiple moving pieces organized.
Requirements
- High School Diploma or equivalent required
- Strong communication, customer service, and problem-solving skills
- Ability to multitask and prioritize in a fast-moving environment
- Professional, positive, and team-oriented attitude
- Strong organizational and computer skills
Nice To Haves
- Previous office, scheduling, staffing, or healthcare experience preferred
- Experience working with elderly, disabled, or medically fragile individuals preferred
Responsibilities
- Coordinate daily schedules for caregivers and nursing staff
- Build and maintain strong relationships with clients and families
- Communicate schedule changes and care updates promptly
- Support and assist field staff with day-to-day needs
- Investigate and resolve client and employee concerns
- Maintain accurate documentation and reporting
- Partner with clinical and operational teams to ensure quality care
- Assist with ongoing quality improvement initiatives
