About The Position

Provides patient care under the supervision of a Case Manager: Nurse or therapist. This position is in home care and requires 100% travel to patients' homes for care, covering Geauga County. The role involves providing personal care, companionship, light housekeeping, and light meal preparation as directed by the plan of care. The Home Care Assistant must effectively communicate verbally and in writing, initiate and maintain positive relationships, observe and report patient status changes, and take responsibility for self-development. A commitment to the mission and values of UHHS/UHHCS is expected. The role also includes performing other duties as assigned, complying with all policies and standards, and abiding by requirements for the safe and secure maintenance of Protected Health Information (PHI).

Requirements

  • High School Equivalent / GED required
  • Completes required continuing education requirements to maintain certification required.
  • Certified Nursing Assistant (CNA) or equivalent certification required
  • Basic Life Support (BLS) Required within 30 Days.
  • OH Driver's License (Valid) with car insurance coverage required.
  • Successful completion of agency written test and competency evaluation of skills in the field Required within 30 Days.

Nice To Haves

  • 1+ years Home care experience preferred.
