We are seeking a compassionate, organized, and detail-oriented Home Care Assistant/Scheduler to join our Home Care team. The Home Care Assistant/Scheduler plays a vital role in supporting the delivery of high-quality home care services to clients in accordance with physician orders, agency policies, industry standards, and applicable insurance guidelines. Working under the supervision of a licensed nurse, this position provides direct patient care while also performing essential administrative and scheduling functions that contribute to the efficiency and success of the Home Care department. This role combines patient support, scheduling coordination, and clerical responsibilities to ensure seamless care delivery, effective communication, and compliance with regulatory requirements. The Home Care Assistant/Scheduler maintains staffing schedules, supports daily office operations, assists with medical record audits, and upholds strict confidentiality in all aspects of the position.

Requirements

  • High School Diploma or GED required.
  • Minimum of six (6) months of supervised work experience as a Nurse Aide in a skilled nursing or acute care setting required.
  • Successful completion of a Certified Nursing Assistant (CNA) program.
  • Current CNA certification in good standing.
  • Current and ongoing completion of all required in-service training.
  • Listed on the state Nurse Aide Registry with no substantiated findings.
  • Must be at least 18 years of age in accordance with Department of Labor regulations.
  • Ability to complete and successfully pass a written and/or skills competency evaluation for Level I In-Home Aide duties, if required.
  • Commitment to completing a minimum of twelve (12) hours of documented in-service education annually during home health employment.
  • Strong organizational, communication, and time-management skills.
  • Ability to maintain confidential information and exercise sound judgment.

Nice To Haves

  • One (1) year of related experience preferred.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Ability to meet EveryAge's automobile insurance carrier requirements for coverage.

Responsibilities

  • Provide quality home care services under the direction and supervision of a licensed nurse.
  • Assist clients with approved in-home aide duties in accordance with established care plans and organizational policies.
  • Promote client comfort, safety, dignity, and independence.
  • Develop, maintain, and coordinate staffing schedules for the Home Care department.
  • Prepare and update assignment sheets, master schedules, and PAL requests.
  • Ensure adequate staffing coverage while responding to scheduling changes and call-offs.
  • Communicate schedule updates promptly to team members and supervisors.
  • Perform a variety of clerical and office management duties that support departmental productivity and efficiency.
  • Schedule meetings and appointments as needed.
  • Maintain department files, records, reports, and correspondence.
  • Facilitate the timely distribution of information and communications.
  • Assist with maintaining standardized operational processes and procedures.
  • Conduct medical record audits to ensure compliance and accuracy.
  • Maintain a high degree of confidentiality regarding patient, employee, and organizational information.
