About The Position

PHMC serves as both a direct service provider to individuals, families, and communities across the region and an intermediary agent, managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment. The HIV Care Manager shall oversee performance, staffing, and funding for HIV care across PHMC Health. This person shall possess exceptional knowledge of Ryan White and/or of Philadelphia Division of HIV Health (DHH) services and shall serve as a liaison to the City, State, and other grant funders around all HIV-treatment and prevention activities. The HIV Care Manager will also be responsible for managing all grants related to HIV treatment and prevention, including compiling written reports and facilitating all necessary health center quality reviews and site visits. The HIV Care Manager shall be responsible for championing and educating health center staff on all Ryan White funding requirements and will work collaboratively with all health center disciplines to ensure patients’ appropriate and continuous access to Ryan White services. The HIV Care Manager will convene regular, interdisciplinary health center meetings to guarantee both coordinated, collaborative care for patients and a consistent review of performance outcomes. The person selected for this role will be responsible for leading process improvement efforts, serving as a spokesperson with external funders, and working collaboratively with members of PHMC leadership and finance teams to ensure the optimal use of program resources. The HIV Care Manager will work to ensure all social service staff are consistently trained on programs available to individuals living with HIV and on programs and services available to persons interested in HIV prevention. The individual chosen for this role will also maintain regular, ongoing, and high-quality supervision of social service staff who work with patients who are either HIV+ or at risk for HIV. The HIV Care Manager shall be responsible, in collaboration with the Medical Director and the Director of Behavioral Health and Social Services, to ensure appropriate documentation of all Ryan White data and information as well as the continual training of staff on Ryan White services. Lastly, the HIV Care Manager will serve as a liaison between PHMC Health and all health center disciplines, City, State, and nonprofit funders, pharmaceutical benefits programs, and local community organizations.

Requirements

  • 2 years of case management and/or care coordination experience required.
  • Master’s Degree from an accredited university or Bachelor's Degree in Social Work, Psychology, Sociology, or other related field with 3 years of infectious diseases, HIV medical case management, PREP, and/or care coordination experience is required.
  • Must be able to effectively communicate with patients, medical providers, interoffice staff, community partners, as well as City, State, and nonprofit funders.
  • Proficient with Microsoft Word, PowerPoint, and Excel.
  • Comfort and flexibility working in a fast-paced health environment.
  • Ability to work independently as well as in a team environment.
  • Must demonstrate sound judgement and initiative.

Nice To Haves

  • 2 years of supervisory experience preferred.
  • Bilingual/Bi-cultural preferred.

Responsibilities

  • Ensure the successful execution of all Ryan White Program requirements and continuous, exceptional performance on Ryan White performance measures.
  • Provide leadership and oversight through facilitated interdisciplinary meetings with all staff engaged in Ryan White-funded activities.
  • Serve as a subject matter expert and spokesperson for outpatient/ambulatory, medical case management, and other PHMC Health Ryan White-funded activities and workflows.
  • Serve as a primary point of contact to all other City, State, and nonprofit funders for all HIV-related initiatives at PHMC Health.
  • Ensure compliance with all DHH and HRSA standards related to HIV treatment, HIV case management, HIV testing, and PREP care coordination.
  • Continuously monitor, analyze, and communicate performance data to the health care team related to all Ryan White performance measures.
  • Provide weekly supervision to the HIV Medical Case Managers and PREP Patient Navigators.
  • Ensure accurate and timely documentation by staff of all client encounters, forms, and applications in the electronic medical record (EMR) and in Careware.
  • Conduct bi-weekly chart outpatient/ambulatory and medical case management chart audits in the EMR and Careware to ensure documentation quality and accuracy.
  • Ensure all DHH training and certification requirements are met.
  • Maintain a strong, up-to-date knowledge of community resources and public benefits.
  • Serve as a liaison between PHMC Health and City, State, and nonprofit funders, pharmaceutical benefits programs, and community organizations that support patients’ expanded access to resources and public benefits.
  • Provide trauma-informed therapeutic support in collaboration with an interdisciplinary team, including crises.
  • Perform other duties as assigned.
  • Keeps abreast of all pertinent Federal, State, and City regulations, laws, and policies as they presently exist and as they change or are modified.
  • Comply with HIPAA and confidentiality policies and procedures as they apply to the job.
  • Comply with Department of Public Health (DPH), The Joint Commission, and other accreditation and regulatory agencies' standards.
  • Adhere to all PHMC policies and procedures.
