About The Position

We are seeking dedicated and clinically sound registered nurses to provide skilled nursing care to patients in their homes. The role includes intermittent visits, private duty shifts, and care plan coordination. This is a flexible per diem role for clinicians licensed in Florida.

The nurse performs skilled nursing visits and shift-based care for patients needing extended clinical support, monitors and documents patient condition and progress, and collaborates with physicians and interdisciplinary teams to update care plans. The role also requires maintaining accurate records in compliance with HIPAA and CMS guidelines, including OASIS documentation for RNs, and upholding infection control standards and agency protocols.

Requirements

• Active Florida RN license; Compact License accepted.
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical completed within the past 6 months.
• Excellent clinical judgment, communication skills, and time management.
• Reliable transportation and commitment to punctual, professional care.

Nice To Haves

• Experience in Medicare-certified home health is strongly preferred.
• Familiarity with EMRs such as Axxess and knowledge of PDGM and OASIS for RNs.

Responsibilities

• Perform skilled nursing visits, including wound care, IV administration, medication management, and patient assessments.
• Provide shift-based care for patients needing extended clinical support.
