About The Position

Lakeview Health Services is seeking a High Needs Supportive Housing Specialist to provide advocacy, linkage, and coordination of services for an assigned caseload. This role focuses on enabling recipients, specifically high-need SMI individuals, to attain and remain in permanent and independent community housing. The specialist will work from a trauma-informed care perspective and a psychiatric rehabilitative approach, striving to create a healing environment that respects the perspectives and experiences of individuals, families, staff, and communities. This involves practicing safe, respectful communication and respecting individual boundaries and differences.

Requirements

Bachelor’s degree in human services (or a related field) or an associate degree and Two (2) years relevant experience.
NYS Driver’s License as driving is an essential function of this position.
Thorough knowledge of community services within service area.
General knowledge and understanding of mental illnesses, psychiatric rehabilitation model, and related issues.
Skills for independent living.
Has a basic understanding of the importance of working from a trauma sensitive perspective.
Must encompass and be able to incorporate key skills to practice and engage high-need SMI individuals which can include; motivational interviewing, suicide prevention, risk screening, trauma informed care, person centered care planning and interventions, and recovery oriented approaches.
Proficient in the ability to improve quality of life and outcomes for high-need individuals with SMI.
Use of contemporary office equipment, particularly a computer with word processing, database and report generating software.
Ability to communicate effectively with diverse individuals.
Ability to listen, understand, and appreciate the experiences of clients.
Ability to persuade, negotiate, and instruct.

Responsibilities

Conduct a comprehensive assessment of needs and develop an individualized support plan.
Maintain engagement with individuals who are often disengaged from care, have difficulty adhering to treatment recommendations, or have a history of homelessness, criminal justice involvement, first episode psychosis, and transitional-aged youth.
Effectively support individuals through skills and practices including but not limited to motivational interviewing, suicide prevention, risk screening, trauma-informed care, and person-centered planning.
Assist residents with identifying and achieving person-centered goals and recovery.
Monitor consumer wellness and ensure well-coordinated care among all providers.
Maintain accurate documentation for all services according to established policies and procedures.
Assist residents with securing and maintaining safe, affordable, permanent housing.
Assist residents in negotiating with landlords, vendors, and service providers.
Assess needs and solicit preferences for residents, make appointments, and inspect housing.
May purchase supplies and physically help with moving, setting up, and maintaining housing.
Maintain accurate records and submit required reports.
Conduct in-home visits at least monthly and review support plans every three months.
Develop and maintain professional relationships with supervisors, colleagues, and community services.
Provide each member of the caseload with a minimum of four contacts per month; at least one contact will be face to face.
Hold a caseload of individuals who have been designated for a high need bed placement such as MRT, RCE, SP-SRO, ESSHI and/or Transformation. The typical caseload will be between 15 and 18 individuals.
May provide more comprehensive case management services as needed.
Participate in county-specific meetings to assure timely processing of referrals for eligible individuals.
