About The Position

Every day, our teams provide compassionate care and support to individuals and families across Connecticut through behavioral health, substance use, domestic housing, and community-based services. We are looking for a HERO Case Manager who is passionate about making a difference and wants to join a team committed to helping people thrive. This is a full time, 37.5 hours per week position in our Housing Program (HERO). Flexible schedule based on client and program needs, including community-based outreach and client support. Travel throughout the Greater New Haven area is required. Candidates must reside in Connecticut to be eligible for this position.

Requirements

  • High school diploma or equivalent required.
  • Experience working with individuals experiencing homelessness, housing instability, substance use disorders, and/or mental health challenges.
  • Understanding of Housing First principles and commitment to person-centered, strengths-based services.
  • Knowledge of housing, employment, and human service resources within the Greater New Haven community.
  • Proficiency in Microsoft 365 and the ability to navigate electronic health record (EHR) systems.
  • Strong communication, relationship-building, and problem-solving skills with the ability to work effectively with diverse populations.
  • Valid driver's license, acceptable driving record, and ability to transport clients as needed.
  • Candidates must reside in Connecticut to be eligible for this position.

Nice To Haves

  • Bachelor's degree preferred.
  • Bilingual English/Spanish skills.
  • Training in harm reduction, motivational interviewing, and de-escalation techniques.
  • Experience working in housing, homeless outreach, behavioral health, substance use treatment, or community-based human services programs.
  • Experience coordinating services and navigating community resources for vulnerable populations.

Responsibilities

  • Build trusting relationships with clients through outreach, engagement, and ongoing support.
  • Help clients identify, obtain, and maintain safe and affordable housing opportunities.
  • Assist individuals in developing personalized goals that promote housing stability, wellness, and independence.
  • Connect clients with employment, healthcare, behavioral health, financial, and community support services.
  • Collaborate with landlords, housing providers, community agencies, and internal BHcare teams to support positive client outcomes.
  • Provide transportation assistance and support clients during transitions or times of increased need.
