About The Position

San Andreas Regional Center (SARC) is one of 21 regional centers in California, funded by the State and rooted in the values of the Lanterman Act. As a community-based nonprofit, SARC is dedicated to supporting individuals with developmental disabilities in living full, independent, and inclusive lives. This position will report to the Health Services Coordinator and assist in representing the health care interests of Individuals receiving services by San Andreas Regional Center. It will act as a coordinator of Health-Services and will assist the HS Coordinator in supporting the providers, Service Coordinators and District Managers. The role requires the ability to analyze data, create reports and spreadsheets to capture department activity, and knowledge of conditions relating to developmental disabilities, Medi-Cal and Medicare, The Department of Developmental Services (DDS), The Department of Social Services (CDSS) and Regional Center operations. A confident public speaker is essential. This position will interface with DDS, Community Care Licensing (CCL/CDSS), Public Health, all Managed Health Plans, all hospitals and skill nursing facilities for all 4 counties and others as needed. This is a 40 hours/week in-office position with occasional weekend and evening required participation for community activities supported by San Andreas Regional Center.

Requirements

  • Licensed RN
  • Minimum of 1 year experience at a regional center or 2 years’ experience at a regional center and/or working with people with developmental disabilities center or similar experience
  • Discharge planning experience
  • Training/teaching experience
  • Excellent communication skills, verbal and written
  • Knowledge of developmental disabilities
  • Ability to work collaboratively with multiple health care systems
  • Good computer skills and knowledge of Google, Excel, PowerPoint and virtual applications like TEAMS and Zoom
  • Use of an automobile, possession of a valid driver’s license and automobile liability insurance for the minimum amount prescribed by law or the ability to provide for independent transportation
  • Knowledgeable of conditions relating to developmental disabilities, Medi-Cal and Medicare, The Department of Developmental Services (DDS), The Department of Social Services (CDSS) and Regional Center operations
  • Confident public speaker

Responsibilities

  • Be familiar with title 17 and 22 as well as Lanterman Act regulations
  • Be a confident public speaker, able to clearly discuss opinion with multidisciplinary professionals, and able to advocate for the Individuals we serve
  • Be an active liaison to all managed care plans for all 4 counties, local community medical providers, DSS, DDS and with the San Andreas Regional Center Quality Assurance Manager to improve quality of services offered to the individuals served by SARC
  • Assist in coordinating health services and discharge plan for individuals served based on the individualized health care plan and needs
  • Help identify all publicly-funded and community resources available or necessary primary and preventive care services as stated in the Lanterman Act
  • Assist in training/educating new SARC employees, Regional Center individuals and their family or support system, Care staff regarding regulations, protocol and policies in place internally and in the community
  • Share responsibility with HSC and DM, to improve information and training to managed health plans, skill nursing or hospital staffs, and others regarding the Lanterman Act and the structure of the Regional Center
  • Serve as a resource for information requested by Individuals, parents, Regional Center Service Coordinators and service providers
  • Work with residential service providers to assure systems are in place for providing current consumer-specific medical information to hospitals or other medical providers, especially when a consumer needs urgent or emergency care
  • Strengthen the training and knowledge of providers of care, by updating training with new regulation found on CDSS PIN, or new DDS requirement or new SARC policies and protocols
  • Participate in various committees internally or in the community to best serve our agency and population served
  • Assist in managing Health Care Services staff
  • Represent San Andreas Regional Center in the community at different official functions
