The Health Services Coordinator will provide support to the health services department to support scheduling needs of individual served by obtaining appointment information from multiple sources of communication including but not limited to televisits, residential staff and RN case managers. This role will be responsible for coordinating necessary paperwork/consults for health services assistant to bring to appointments supporting. Further, will provide the health services assistant with their weekly schedule and assist in coordinating around urgent needs in providing health assistant support to individuals. After the appointment is completed this role will collect follow up appointment needs for scheduling purposes from the health services assistant.

Requirements

  • Proof of HS Diploma or GED education
  • 2 years’ experience supporting individuals with medical needs
  • Experience with Microsoft word suite including Excell and Word programs
  • Verbal and written communication skills
  • Organized and proficient at meeting deadlines

Nice To Haves

  • Knowledge of CaraSolva Electronic Medical Record system
  • 1 year of experience working in an OPWDD regulated organization

Responsibilities

  • Consistently align and demonstrate the agency’s values while performing all job duties
  • Obtain and maintain weekly or monthly appointment calendars from each program, including televisits
  • Communicate regularly with the RN Case Manager to identify anticipated MIPPS needs prior to appointments
  • Ensure the Medical Consultation Form is completed prior to every appointment
  • Enter appointment prompts into CaraSolva
  • Promptly notify the Pharmacy & Clinical Operations Administrator, Nurse, Deputy, and Health Services Director via email of Medication changes or Newly prescribed treatments
  • Collect MAOS documentation daily from residential staff and/or the Health Services Assistant
  • Schedule appointments that are identified as necessary but were not scheduled during the patient’s visit
  • Notify residential staff of scheduled appointments so they can add them to the house calendar
  • Collaborate with the RN Case Manager and Residential House Manager to ensure all appointments are scheduled in a timely manner and to identify any support needs that may require assistance from the Health Services Assistant
  • Identify necessary house resources, including staffing availability and vehicle access
  • Collaborate and strategize with the RN Case Manager and Residential House Manager to resolve appointment conflicts or missed appointments requiring rescheduling
  • Identify and notify the RN Case Manager of high-risk or difficult-to-obtain appointments, coordinating with residential leadership as needed
  • Assist RN Case Managers with post-hospital follow-up scheduling when an individual is discharged from the hospital or emergency room
  • Work collaboratively with the Pharmacy & Clinical Operations Administrator to support pharmacy management needs and serve as a backup as necessary
  • Perform other duties as assigned by the Pharmacy & Clinical Operations Administrator
