About The Position

The Health-Related Social Needs (HRSN) Coordinator plays a critical role in supporting individuals experiencing housing instability or homelessness by helping them access housing-related benefits, services, and resources. This position serves as the administrative backbone of the HRSN housing program, coordinating referrals, managing documentation, processing invoices, and ensuring timely delivery of housing support services. The HRSN Coordinator primarily works in an office setting, communicating with participants, landlords, housing providers, care teams, and community organizations. The Coordinator helps facilitate tenancy support services, housing navigation, and benefit administration while maintaining compliance with program requirements and funding guidelines. The Coordinator combines strong housing knowledge with a commitment to improving health outcomes through housing stability and equitable access to services. This role is ideal for someone with a strong housing-services background who understands housing programs, tenancy processes, landlord engagement, and housing-related administrative workflows. This is a primarily administrative, office-based role focused on coordination, documentation, and referral management. The position does not involve direct, in-person outreach or face-to-face client services, though it supports staff and partners who provide those services.

Requirements

  • Bachelor's degree in Social Work, Human Services, Public Health, Housing Studies, or a related field; or equivalent combination of education and relevant experience.
  • Experience working in housing services, homelessness systems, tenancy support, care coordination, social services, or a related field.
  • Excellent organizational, administrative, and documentation skills.
  • Ability to manage multiple priorities and maintain attention to detail in a fast-paced environment.
  • Proficiency with computer systems, databases, Microsoft Office applications, and electronic case management platforms.
  • Valid driver's license and ability to travel as needed.
  • Must successfully pass pre-employment and/or for-cause drug screening.
  • Must successfully pass criminal background and DMV checks.

Responsibilities

  • Identify individuals eligible for HRSN benefits and services.
  • Develop individualized housing support plans in collaboration with participants, care teams, and housing partners.
  • Coordinate referrals to housing providers, supportive programs, and community-based organizations using closed loop referral software.
  • Provide housing navigation and tenancy support services.
  • Manage a weekly caseload provided by the HRSN Manager, and collaborate with other HRSN Coordinators on.
  • Conduct ongoing follow-up to support housing stability and continuity of care.
  • Complete invoicing and documentation for housing-related HRSN services.
  • Serve as a liaison between care coordination teams, housing agencies, and community partners.
  • Educate participants on housing resources, program expectations, and participant responsibilities.
  • Maintain working knowledge of and provide referrals to community services and resources, including Aging and Disability Services, Housing Assistance, Transportation, Food/Shelter Services, and Public Health resources.
  • Provide culturally informed support to staff working directly with clients to ensure culturally responsive service delivery.
  • Performs other duties as assigned.
