About The Position

The Greater Richmond YMCA is seeking part-time Social Needs Navigators to work with their chronic disease health programs. The Social Needs Navigator collaborates with internal YMCA team members and external members of a patient's care team. They accept referrals, manage assigned caseloads, and develop care initiatives to assess and coordinate care management for patients. The Navigator acts as a vital link, guiding and supporting patients through and around barriers to lifestyle change. Candidates must be available to work 20-25 hours per week, which may include nights and weekends with occasional weekend availability.

Requirements

  • At least 18 years of age.
  • Experience working in behavior change, wellness, specialized populations, or social work.
  • Must possess health coaching skills and Listen First/Motivational Interviewing skills.
  • Ability to travel to YMCAs within the Metro area.
  • Proficiency in Microsoft suite and ability to multi-task on various digital platforms.
  • Must comply with organizational compliance and privacy policies.
  • CPR/AED/First Aid certification required upon hire.

Nice To Haves

  • Bachelor's degree in Human Services field.
  • Bilingual with Social Services, mental Health, or Case Management Background.
  • Community Health Worker training preferred.
  • College degree in Social Work, Counseling, Public Health, Sociology, Health Education or life experience equivalent.
  • Experience in case management or social work.
  • Skills in motivational interviewing/health coaching.
  • Experience in data collection and entry.
  • Bilingual candidates preferred.
  • Bilingual Language Preferred.
  • Transportation - this position will require in-person engagement at multiple locations.

Responsibilities

  • Connecting individuals to services that address basic needs including housing, healthcare, and food.
  • Connecting individuals to community health programming within the YMCA and offerings within the community.
  • Conducting in-person or small group coaching as scheduled.
  • Accepting referrals and managing assigned caseload.
  • Developing care initiatives to assess and coordinate care management for patients.
  • Serving as an essential link, guiding and supporting patients through and around barriers of lifestyle change.
