About The Position

The Health Educator will facilitate, implement, present, and evaluate health education programs and services to patients of all ages. This role involves assessing patient needs, establishing priorities, and developing, recommending, updating, implementing, and evaluating health education programs and services. The educator will provide group class or one-on-one health education services and conduct individual and small group health coaching. They will counsel patients on a variety of subjects including diabetes, hypertension, asthma, smoking cessation, substance abuse, teen health, Hepatitis B, preventive cancer screening and immunization, proper nutrition, and healthy lifestyle options. The position also requires coordinating a wide range of health education services, supporting program development to improve health outcomes, and developing culturally appropriate written and audio-visual health education materials. Community outreach and education activities are also a key part of the role, as is collaborating with other departments on health information projects. The Health Educator will assist in leading summer interns and volunteers, serve as a resource to patients, collect and analyze program data, prepare monthly service reports, and provide phone call services for appointments and events.

Requirements

  • High school diploma with 4 - 8 years equivalent experience required
  • Minimum of two (2) years working experience in health education field and/or health care setting
  • Ability to facilitate wellness and exercise classes for older adult patients
  • Current Group Fitness Certification from a nationally recognized fitness training organization (i.e. ACE, AFAA, NASM, ACSM) or ability to obtain it
  • Current National Board-Certified Health & Wellness Coach (NBC-HWC) Certification or ability to obtain it within 6 months of first available training date is required
  • Strong organizational and time-management skills
  • Ability to work a flexible schedule in a variety of settings
  • Knowledge of community resources
  • Ability to present ideas accurately, concisely, and effectively both orally and in writing; assist in the promotion of public health education programs
  • Proficient computer skills including use of Microsoft Office and Outlook
  • Current documentation of Basic Life Support is required
  • Fluent in Chinese (Cantonese), both oral and written
  • Must be able to read and write English fluently

Nice To Haves

  • Bachelor’s degree in Health Education, Public Health, Health Science or related field
  • Certified Clinical Exercise Specialist Training
  • 1-2 years of group fitness teaching experience
  • Fluent in Mandarin

Responsibilities

  • Coordinates a wide range of health education services, including recruiting members, establishing referral system, providing scheduling and logistical support to promote the well-being of patients and the community
  • May be required to work sometimes at satellite NEMS sites and/or at weekend events
  • Supports development, implementation and evaluation of health education programs and activities to improve health outcomes
  • Develops and recommends high quality, culturally appropriate, written, and audio-visual health education materials
  • Performs relevant community outreach and education activities to targeted groups and individuals
  • Works collaboratively with Administration and Graphic Design Department and assists in health information projects such as activity schedule, newsletters, informational displays, flyers, posters, staff bulletin, community health events
  • Assists in leading summer interns and volunteers and implementing education activities for the Health Center Week
  • Serves as a resource to patients and provides them with the best information available
  • Collects, tabulates, and analyzes program data, and prepares monthly services reports
  • Provides phone call services and schedules appointments for patients for the events and services such as classes and support groups
  • Performs other job duties as required by manager/supervisor
