The Health Education Program Coordinator manages and implements health education programs, develops community education and outreach activities, and counsels patients. This role involves developing, implementing, coordinating, and evaluating bilingual health education programs and activities on preventative health care, chronic disease management, Western medicine, and other related topics. The coordinator will also develop curricula, toolkits, and health education materials, provide group and one-on-one health education services, and act as a liaison with community agencies and schools. Additionally, the role includes managing Health Education centers, counseling patients on various health subjects, coordinating with clinical departments for multilingual educational material, researching grants, analyzing program data, and collaborating with administration on health information projects. The position requires travel to clinic sites, supervision of interns and volunteers, and providing operational support to Health Education Assistants. The coordinator will also administer Remote Patient Monitoring, make outreach calls, assist with ad hoc outreach, and cover other sites as needed.
