HEALTH EDUCATION PROGRAM COORDINATOR
About The Position
The Health Education Program Coordinator manages and implements health education programs, develops community education and outreach activities, and counsels patients. This role involves developing, implementing, coordinating, and evaluating bilingual health education programs and activities focused on preventative healthcare, chronic disease management, Western medicine, and other related topics. The coordinator is responsible for creating curricula, toolkits, and educational materials, and providing both group and one-on-one health education services. A key aspect of the role is acting as a liaison with community agencies and schools to foster outreach programs, managing Health Education centers, and counseling patients on various health issues. The position also entails coordinating with clinical departments for multilingual material development, researching grants, analyzing program data, and collaborating with administrative departments on health information projects. Travel to clinic sites is required for program delivery. The role includes supervising interns and volunteers, providing operational support to Health Education Assistants, administering Remote Patient Monitoring, and performing other duties as assigned.
Requirements
- Bachelor’s degree (degree in public health or related field preferred).
- Health & Wellness Coach Training completed by a NBC-HWC-approved program or ability to obtain it within 6 months of first available training date is required
- Current documentation of Basic Life Support is required
- Must be fluent in English (reading, writing, and speaking)
- Must be fluent in Chinese: Cantonese and/or Mandarin (reading, writing, and speaking)
Nice To Haves
- CHES certification preferred
- Two years’ experience in community health education, health communication or related field preferred
- Prior experience in program planning, design and implementation that incorporates evidence-based strategies and cultural competency
- Experience with formative research or program evaluation preferred
- Prior experience working with the underserved, immigrant, low-income and/or Asian communities preferred
Responsibilities
- Develops, implements, coordinates, and evaluates bilingual health education programs and activities on preventative health care, chronic disease management, Western medicine, and other related topics
- Develops curricula, toolkits, health education programs and materials
- Provides group class and/or one-on-one health education services to members and the general public
- Acts as primary liaison with appropriate community agencies and schools to establish, promote, and carry out community education and outreach programs
- Manages Health Education centers
- Counsels patients on a variety of subjects such as diabetes, hypertension, asthma, smoking cessation, substance abuse, teen health, Hepatitis B, preventive cancer screening and immunization, etc.
- Coordinates with clinical departments to develop multilingual educational material
- Reviews translations of materials for accuracy and appropriateness
- Researches grants and funding opportunities, and assists in writing grant proposals
- Collects, tabulates, analyzes program data, and prepares reports
- Works collaboratively with Administration and Project Management Department, and provides support in health information projects such as activity schedule, newsletter, informational displays, flyers posters, staff bulletin, community health events, etc.
- Travels to NEMS’ clinic site to conduct programs and events
- Supervises summer interns and volunteers and assists in implementing education activities for the Health Center Week
- Provides day-to-day operational support to Health Education Assistants including scheduling, training, and guidance; acts as a liaison to communicate updates or concerns to Health Education Manager as needed
- Supports walk-ins and provider referrals to Health Education
- Administers Remote Patient Monitoring assignment, monitoring, tracking and calls
- Calls outreach lists tied to organizational goals and PIP measures, as needed
- Assists with ad hoc outreach lists (e.g., health plan initiatives or incentive programs that require patient outreach), as needed
- Covers and rotates to other sites as required
- Performs other job duties as required by manager/supervisor
