About The Position

This position is responsible for delivering evidence-based curricula to at-risk youth and adults in the childhood obesity intervention program, Take C.H.A.R.G.E., and the diabetes prevention program, Lifestyle Balance. The Health Coach will provide coaching and support to participants throughout the intervention program using evidence-based strategies. Duties include using health-related assessments and tools to develop health improvement plans for at-risk youth and/or adults. The incumbent will assist the Diabetes Prevention Coordinator in monitoring and evaluating participant health outcomes and other activities supporting SDPI grant goals and objectives. Experience with electronic health records and knowledge of local tribal philosophy and cultural practices are essential. The incumbent must be willing to work evenings and weekends as needed. This position is grant-funded.

Requirements

  • Bachelor’s degree in Exercise Science, Physical Education, Kinesiology, Nutrition, or similar health-related field
  • One (1) year demonstrated experience in the area of health education or coaching principles, practices, methods, implementation, and evaluation of preventive health service programs.
  • Must have one (1) year experience in diabetes prevention programs for at-risk youth /or adults or related health programs.
  • Demonstrated experience in behavior change, motivational interviewing, basic nutrition and fitness principles
  • Must have and maintain a Basic Life Support Certification (BLS) from the American Heart Association (AHA) or must obtain within 6 months of hire.
  • Training or Certification in Group Fitness or Personal Fitness or Must obtain within 6 months of hire
  • Must maintain and have an unrestricted current valid state driver’s license
  • A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas: Positive working relationships with others, Possession of high ethical standards and no history of complaints, Reliable and dependable; reports to work as scheduled without excessive absences.
  • Basic knowledge data collection, analysis, practice, and ethical considerations
  • Proficient with the following software programs: Excel, Word, Power Point, Outlook, Adobe, and/or Access
  • Skilled and comfortable in conducting presentation/public speaking.
  • Must maintain and have an unrestricted current valid state driver’s license.
  • Training in Lifestyle Balance curricula and other evidence-base prevention curriculum for at-risk youth and/or adults.
  • Completion of and, above-satisfactory scores on all job interviews, demonstrating to the satisfaction of the interviewees and TCRHCC that the applicant can perform the essential functions of the job
  • Successful completion of and positive results from all background and reference checks, including positive employment references from authorized representatives of past and current employers demonstrating to the satisfaction of TCRHCC a record of satisfactory performance and that the applicant can perform the essential functions of the job
  • Successful completion of fingerprint clearance requirements, physical examinations, and other screenings indicating that the applicant is qualified to be employed by TCRHCC and demonstrating to the satisfaction of TCRHCC that the applicant can perform the essential functions of the job
  • Submission of all required employment-related documents, applications, resumes, references, and other required information free of false, misleading, or incomplete information, as determined by TCRHCC.

Responsibilities

  • Provide health coaching and support to Take C.H.A.R.G.E. Program and Lifestyle Balance Program participants through the delivery of evidence-based curricula in individual or group settings.
  • Incorporate and integrate culturally appropriate concepts, modifications and activities when delivering health promotion/diabetes prevention education and services.
  • Administer initial and follow-up health assessments, including but not limited to: blood pressure, pulse readings at rest, during and post exercise; FitnessGram administration; aerobic capacity testing, body composition measurements, and tests for muscular strength, endurance and flexibility. Other tests may be conducted to monitor blood glucose levels prior to, during and post exercise sessions.
  • Assist with referrals and scheduling coaching sessions with all patients/participants of Take C.H.A.R.G.E. and Diabetes Prevention Program clinics and those referred from other clinics/programs of TCHRCC.
  • Assist in reminder calls and follow-up coaching communication as needed to support participant recruitment, participation, and retention.
  • Assist participants in setting personal health (health-related fitness, weight loss, etc.) goals and motivate them to adopt healthy behaviors. Utilize active listening skills to identify issues/needs and provides appropriate solutions or referrals.
  • Orient and instruct individuals in proper use of exercise equipment and techniques in exercise. Develop and ensure a safe environment for participants and community members utilizing the fitness center.
  • Identify and utilize appropriate resources for participants. Work closely with the Diabetes Prevention Coordinator and other health coach/es in the delivery of Take C.H.A.R.G.E. and Lifestyle Balance Program classes to meet health outcomes.
  • Assist with Take C.H.A.R.G.E. Program and Lifestyle Balance Program registry for grant and health outcome reports.
  • Apply knowledge of behavior change theories and models to meet participant health goals.
  • Work with the care team (medical providers, health coach/es, etc.) on the delivery of intervention plans for participants in the Take C.H.A.R.G.E. Program and Lifestyle Balance Program.
  • Attend meetings as it relates to the grant or secondary prevention programs.
  • Utilize appropriate media/marketing techniques in the delivery of programs to increase program visibility. Assist with media services/social marketing.
  • Deliver effective communication toward supervisor, program staff, program partnerships, hospital staff, and community members. Communication abilities include written and verbal skills that demonstrate the ability to motivate or encourage others to alter lifestyles and group dynamics.
  • Foster partnerships within TCRHCC as well as with outside organizations. Provide assistance to other Service Unit HP/DP activities. Work with other departments and/or clinics for collaborative purposes and to meet common goals.
  • Provide outcome reports and analysis for presentation; includes mandatory reporting.
  • Understand and adhere to established compliance, safety, and quality assurance standards.
  • Assist Community Connectors by providing distribution of food, water and other necessary essentials to community members who are in isolation or quarantine.
  • Assist the Contact Tracing team by performing contact tracing activities for patients testing positive for COVID-19, and conducting negative results calling.
  • Assist the Screening/Greeters at various entry points to ensure appropriate screening for all patients and visitors entering the TCRHCC campus.
  • Ensure proper PPE is worn at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.
  • Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly.
  • Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction.
  • Perform other duties as assigned.
