About The Position

The Health and Wellness Director oversees the day-to-day operations of the health and wellness department and participates in planning, implementing, and evaluating resident care. The director is responsible for department compliance, assessments, care planning, direction, budgeting, staffing, and hands-on care in accordance with applicable regulations and company policies. The position reports to the Executive Director and is classified as exempt and salaried. The director leads the health and wellness team, monitors resident care and outcomes, manages infection control and quality assurance, and coordinates support services. The role includes hiring, training, and mentoring team members, maintaining staffing and budget targets, assisting with nursing coverage as needed, and responding to on-call needs. The position requires weekend and holiday availability, 24/7 on-call responsibilities, and occasional weekend coverage as Manager on Duty.

Requirements

Graduate of an accredited school of nursing with a current, valid Registered Nurse (RN) license; for Massachusetts and Pennsylvania communities, a current, valid Licensed Practical Nurse (LPN) license may be accepted as permitted by applicable state regulation.
Four or more years of supervisory experience in Senior Living, Assisted Living, Long-Term Care, or Skilled Care.
Proficiency in computer skills and Microsoft Office, including Windows, Outlook, and Excel, with the ability to learn new applications.
Able to work weekends and holidays.
Ability to perform physical activities including standing, walking, sitting, using fine and gross motor skills, reaching, balancing, stooping, kneeling, crouching, talking, hearing, and smelling.
Ability to lift or carry up to 50 pounds; intermittent lifting and supporting of residents is required, using proper body mechanics.

Responsibilities

Oversee day-to-day operations of the health and wellness department, including compliance, planning, assessments, direction, budgeting, staffing, and hands-on care.
Conduct thorough resident assessments.
Ensure resident care needs are met and maintained according to physician orders, state regulations, and company policies and procedures.
Monitor the progress and effectiveness of resident care plans through written progress notes and verbal communication with residents and associates.
Evaluate health and wellness associates’ skills and overall performance.
Manage infection control and QAPI, including communicable disease reporting, monitoring, and surveillance.
Participate in QAPI and determine root causes of resident data and outcomes.
Complete nursing audits as needed, including audits of medication carts, medication boxes and storage, and eMARS/TARS.
Implement training programs and in-services for associates in accordance with state regulations.
Provide leadership to health and wellness associates and ensure staffing levels and shift schedules operate effectively.
Hire, train, and mentor Wellness Nurses and Health and Wellness Team Members.
Coordinate ancillary support services for residents.
Inform physicians, families, and associates of changes in residents’ conditions and needs.
Maintain the department within budgetary guidelines for staffing and supplies.
Assist with nurse coverage as needed.
Fulfill 24/7 on-call responsibilities and provide occasional weekend coverage as Manager on Duty.
Participate in and attend scheduled in-service training.
Ensure compliance with federal, state, and local regulations and community policies and procedures.
Perform other duties as assigned.
