About The Position

HomeWorks, the home care division of United Methodist Communities, is hiring a per diem Geriatric Care Manager to serve clients across Collingswood, New Jersey. The role involves in-home assessments, personalized care planning, advocacy, service coordination, and support through healthcare transitions. The position pays $50 per hour and offers flexible, as-needed day-shift scheduling. Mileage is reimbursed between client visits. Applicants must be able to produce a negative drug test and may be subject to a background check.

Requirements

RN license, LCSW/MSW license, or equivalent human services professional credential.
At least 3 years of experience in geriatric care management, aging services, home care, or case management.
Valid driver's license and reliable transportation.
Ability to produce a negative drug test.

Nice To Haves

Care Management Certification (CMC) or Aging Life Care Professional® certification.
Working knowledge of Medicare, Medicaid, and long-term care planning.
Experience with older adults and clients with chronic illness, dementia, or disability.

Responsibilities

Conduct in-home client assessments and create personalized care plans that support health, independence, and quality of life.
Connect clients with healthcare services, community resources, and support programs suited to their needs.
Advocate for clients and families in healthcare decisions.
Coordinate with physicians, providers, caregivers, and community partners.
Support clients through transitions such as hospital discharge and placement in rehabilitation, assisted living, or skilled nursing.
Monitor client progress and adjust care plans as needs change.
