About The Position

The Geriatric Care Coordinator serves as a link between elderly patients, their families, and the multidisciplinary care team. The role supports patient navigation, administrative tasks, and coordination of care services to contribute to quality patient outcomes. The coordinator guides patients through basic access-to-care steps by confirming needs, documenting requests, sharing standard information, and routing questions to the appropriate care team or service area. The role coordinates routine appointments, referrals, transportation, community resources, and follow-up using established workflows and approved communication practices. The position performs routine clinical support tasks under close supervision, using established checklists and protocols. Decisions follow clearly defined procedures and have limited impact beyond the employee’s own work. Work is reviewed routinely by licensed clinical staff. Responsibilities include scheduling and confirming appointments; communicating with patients, family members, providers, and care teams; coordinating post-discharge visits and referrals; enrolling eligible patients in Advanced Primary Care Management and tracking associated work; managing GUIDE Program referrals and visits; and screening and returning family calls on behalf of the Nurse Practitioner. Administrative work includes patient registration and data entry, maintaining provider census and tracking information, verifying insurance and obtaining authorizations, documenting after-hours calls, and assisting with practice capacity, letters, and provider requests. Clinical support includes processing lab, radiology, and Durable Medical Equipment orders; assisting with medication refills and orders; sending medical records and clinical documentation; and managing specified compliance-related requests. Millennium Physician Group was formed in 2008 and is headquartered in Fort Myers, Florida, with offices in Florida, North Carolina, Georgia, and Texas. The company describes its employees as the foundation of its success and says it provides tools, a team atmosphere, and opportunities to improve care and grow in the role.
