About The Position

The Gateway Transition Coordinator is responsible for the initial response to the referral, completion of the Return to Community Assessment (RTCA) and Options Counseling, obtaining Gateway to Community Living (GCL) Informed Consent, and initiating the Individual Transition Plan (ITP) under the Alabama Community Living (ACL) Medicaid Waiver Home and Community-based Services Program (HCBS) within the SARPC area and surrounding counties following specific requirements as set out in ACT Medicaid Waiver guidelines.

Requirements

• Knowledge of social work programs and processes preferred.
• Knowledge of community resources and support network available to clientele served.
• Excellent communication skills, both orally and written.
• Excellent organizational and time management skills.
• Ability to relate to the elderly and their unique problems.
• Working knowledge of Medicaid eligibility and Nursing Home requirements preferred.
• Knowledge and ability to operate office equipment including telephone, copy machine, fax machine, computer, and calculator.
• Knowledge and ability to do basic arithmetic.
• Ability to read and interpret guidelines and regulations and apply them to the job.
• Bachelor’s Degree from an accredited four-year college or university in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or a related field.
• At least one (1) year of case management experience.
• A valid driver’s license and a good driving record.

Responsibilities

• Serve as the first line for individuals who seek to transition from an institutionalized care setting back into the community under the Alabama Community Transitions (ACT) Waiver / Gateway to Community Living (GCL).
• Responsible for initial response to the referral, completion of the Return to Community Assessment (RCA), Options Counseling, and obtaining GCL Informed Consent.
• Initiate the Individual Transition Plan (ITP) through a person-centered process, ensure the waiver Plan of Care is integrated into the ITP, and is responsible for overall tracking of the ITP through completion of implementation.
• Work closely with the participant, the nursing facility discharge planner, and the waiver case manager to monitor and facilitate the overall implementation of the ITP and its projected timeframes.
• Ensure the GCL Office is informed of projected move/transition dates in a timely manner and ensure completion of a Quality of Life (QoL) Survey is completed by the local Ombudsman.
• Market and perform outreach regarding the ACT Waiver and transition procedures to include one visit monthly to every regional facility.
• Ensure compliance with state and federal standards and regulations.
• Attend conferences, seminars, and other continuing education programs as required.
• Other duties as required.
