About The Position

Community Care is seeking a Functional Eligibility Screener to support its mission of helping older adults and adults with physical and intellectual disabilities live as independently as possible in the community. This full-time position is based at the Kenosha location and is suited to someone experienced with the LTCFS assessment tool who enjoys training and supporting staff. The screener performs functional eligibility assessments, helps certify and train staff, and provides oversight of screening activities. The role includes regular local travel using a personal vehicle.

Requirements

Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
Current Long-term Care Functional Screener certification.
At least 18 months of experience in a social service capacity, or a master's degree in Social Work, Psychology, or Human Services and one year of experience with older adults or people with physical and/or intellectual disabilities in a social service capacity.
Valid driver's license and proof of required insurance coverage.

Responsibilities

Perform initial, periodic, and change-of-condition functional screens for Family Care, PACE, and Partnership Program members.
Provide ongoing training to certified staff to help maintain their skills and incorporate new information into practice.
Develop, provide, and track LTCFS training programs for new employees.
Assist new Care Management staff in obtaining LTCFS certification.
Provide intensive oversight of LTCFS activities for a minimum of six months to ensure screeners are competent.
Track timely completion of screens in the computer system.
Participate in team meetings.
Travel locally as needed, using a personal vehicle.
