About The Position

Family Support Services (FSS) includes multiple programs serving pregnant and parenting adolescents and families in Southern Alameda County (Hayward, Union City, Newark and Fremont). Programs include: Targeted Case Management (TCM) Cal-Learn Alameda County Public Health Home Visiting Program (ACPH) Targeted Case Management (TCM): TCM provides Medi-Cal-eligible individuals with case management services to improve access to medical, social, educational, and other supportive services. Core services include needs assessment, setting needs objectives, individual services planning, service scheduling, crisis assistance planning, and periodic evaluation of service effectiveness and ongoing client needs. Cal Learn Program: Cal Learn supports pregnant and parenting teens receiving CalWORKs/TANF in completing high school or equivalent education, achieving self-sufficiency, and strengthening family stability. Services include case management, support for school engagement, and assistance with transportation, childcare, and school supplies. Alameda County Public Health Home Visiting Program (ACPH): ACPH Home and clinic visiting program provides comprehensive, family-centered case management for pregnant and parenting individuals with children up to age five. Services focus on child development, health, school readiness, and family well-being, delivered in homes, clinics, schools, and community settings. The Family Support Services Case Manager I plays a critical role in improving health, educational, and life outcomes for vulnerable youth and families in the community. The position provides comprehensive, trauma-informed case management services to pregnant and parenting adolescents and families. The role works as part of a multidisciplinary care team to deliver integrated health and social services, promote positive child and family outcomes, and support clients in achieving individualized goals. Services are delivered in clinics, homes, schools, and community settings. Schedule: This is a full-time position, working 40 hours per week, typically Monday through Friday, with occasional Saturday hours. Requirements

  • Bachelor’s degree in social work, psychology, or human services; OR Associates degree with two (2) years of case management experience in health and human services; OR Minimum of four (4) years of case management experience in health or human services.
  • Experience working with adolescent population.
  • Bilingual in English/Spanish required.
  • Strong commitment to working with diverse adolescent and family populations.
  • Cultural humility and sensitivity to diverse family structures and experiences.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills and ability to communicate effectively and build effective working relationships.
  • Ability to clearly document service plans, reports, memos, etc.
  • Ability to maintain accurate documentation and meet deadlines.
  • Knowledge of confidentiality standards and ethical practices.
  • Knowledge of trauma-informed care principles.
  • Knowledge and understanding of health information related to parenting and pregnancy.
  • Able to work independently and collaboratively in a team environment.
  • Ability to work on an interdisciplinary basis with school system and health teams.
  • Able to meet or exceed the Service Excellence Standards of TVHC, Inc.
  • Ability to travel between various sites and locations as directed by the program.

Responsibilities

  • Provide case management services to up to 28 families per month in accordance with program and contract requirements.
  • Develop individualized service plans addressing health, psychosocial, educational, vocational, and financial goals.
  • Monitor client progress and adjust service plans as needed.
  • Conduct regular client engagement (minimum of two contacts per month).
  • Conduct psychosocial assessments with pregnant and parenting clients.
  • Administer depression, developmental and Social Determinants of Health (SDOH) screenings.
  • Identify client needs and coordinate appropriate internal and community referrals.
  • Provide supportive counseling and parenting education as needed.
  • Conduct home visits to maintain engagement and assess the Family’s living environment. Telehealth visits are also conducted as needed.
  • Link clients to the school systems including high school completion, continuation programs, and vocational programs.
  • Assess eligibility and coordinate supportive services (childcare, transportation and ancillary services for Cal Learn clients).
  • Educate clients on pregnancy, child development, parenting, nutrition, family planning and educational options.
  • Ensure coordination of and access to TVHC Medical, Dental, Behavioral Health, and Specialty care services including TVHC wellness programs as needed.
  • Collaborate with TVHC providers as needed to offer integrated services for families enrolled.
  • Participate in individual and group supervision by presenting client cases to assess needs and develop interventions.
  • Maintain active referral network of community resources and services for clients within Southern Alameda County.
  • Facilitate individual and group support sessions in person or virtual in clinics, schools, or community settings.
  • Assist with outreach to identify and enroll eligible clients into the program.
  • Maintain accurate, timely, and confidential case documentation.
  • Participate in chart audits and quality assurance monitoring
  • Ensure compliance with program standards and contractual requirements.
  • Participate in internal and external trainings.
  • Support program activities and community partnerships.
  • Perform other duties as assigned.
