Foster Care Case Manager
About The Position
Wellspring Lutheran Services is hiring a full-time, salaried Foster Care Case Manager serving Kent, Muskegon, and Kalamazoo counties and surrounding West Michigan communities. The role provides consistent support to children and families navigating foster care, helping them move toward safety, stability, and permanence. The case manager works in the community with children, foster parents, birth families, schools, courts, therapists, and other service providers. Training, onboarding, and early support are provided by the experienced Kentwood-based team. The position combines structured case management with direct, compassionate support. The case manager develops individualized plans, maintains regular contact through monthly home visits and ongoing communication, connects families with resources, advocates for children, coordinates family visits, responds to crises, and maintains accurate records. The role includes participation in a shared on-call rotation, which may require availability beyond the typical workday.
Requirements
- Bachelor’s degree in social work or a related human services field.
- Valid Michigan driver’s license and reliable transportation.
- Empathy, accountability, and professional ethics.
- Ability to balance independent work with teamwork.
- Clear communication and thoughtful problem-solving skills.

Responsibilities
- Develop individualized plans with children and families, building on their strengths.
- Conduct monthly home visits and maintain consistent communication with children and families.
- Help families access healthcare and community support resources.
- Coordinate with schools, therapists, foster parents, courts, and other people involved in a child’s care.
- Prepare for hearings and advocate for each child’s well-being and voice.
- Coordinate visits between children and their parents, siblings, and loved ones.
- Provide calm, capable support during crises.
- Maintain clear and accurate records.
- Participate in a shared on-call rotation.
