The UNC Formerly Incarcerated Transition (FIT) Program is based in the Department of Family Medicine at UNC Chapel Hill. It partners with the North Carolina Department of Adult Correction, Federally Qualified Health Centers, county health departments, community-based organizations, and local reentry councils to connect people returning from incarceration with essential health services and other reentry resources. Community Health Workers with prior justice system involvement act as peer navigators, helping clients establish primary care and access mental health and substance use disorder treatment, as well as assistance with housing, employment training, legal needs, education, transportation, life skills, and interactions with the justice system. The Wake County FIT Operations Supervisor is a hybrid position responsible for day-to-day operational management of Wake County FIT and FIT Wellness services. FIT Wellness provides integrated psychiatric and primary care services to people with serious mental illness returning to the community after incarceration. The supervisor directly oversees currently five Community Health Workers and manages daily workflows, clinical operations, staff development, and implementation of program policies to support high-quality, patient-centered care. The role is the operational lead for Wake County FIT and FIT Wellness and collaborates with FIT Program leadership on quality improvement, program implementation, and operational planning.
