About The Position

Mass General Brigham Home Hospital program redefines acute care delivery by providing hospital-level care directly in the patient’s home. This offers comfort, dignity, and personalized attention while helping reduce hospital stays and improve outcomes. The role combines the autonomy of community-based nursing with the clinical challenge of caring for patients with acute, inpatient-level needs. You’ll work with a supportive interdisciplinary team and participate in innovative care models designed to improve access, quality, and patient experience. Team members are asked to commit to a minimum of 4 shifts per month (8:00 AM – 8:30 PM), with two of those falling on a weekend day. This position supports patients across the South/West Region, including Boston and its residential areas, as well as Framingham, Natick, Newton, Dedham, Quincy, Sharon, and nearby communities. We provide hospital-level care for a diverse adult patient population, including those with acute medical conditions, post-ED or post-hospitalization needs, infections requiring IV therapies, chronic disease exacerbations, and diagnostic follow-up, remote monitoring, and daily clinical oversight. You’ll deliver care traditionally provided in an inpatient setting—right in the patient’s home.
