About The Position

Concierge Home Care is seeking a Home Hospital Registered Nurse (RN) in Jacksonville, FL. The Home Hospital RN provides higher-acuity skilled nursing care directly in patients’ homes, supporting recovery, independence, and safe healing. This specialized division does not require Medicare OASIS documentation, allowing more focus on patient care and clinical treatment.

Requirements

Active, unencumbered Florida RN license
Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

Home health experience preferred but not required

Responsibilities

Conduct patient assessments and develop care plans
Provide advanced skilled nursing treatments and patient education
Coordinate care with physicians and interdisciplinary teams
Monitor patient progress and adjust care plans as needed
Document visits efficiently using AI-assisted tools
TPN administration
Chest tube management
Lab draws
IV therapy and maintenance
Wound vacs and wound care
Post-surgical recovery support
Respiratory care
Patient and caregiver education
