About The Position
Activa Home Health is seeking a Field Physical Therapist Assistant to provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist. The role involves implementing individualized treatment plans to improve patients’ mobility, strength, balance, and function; supporting therapeutic exercises and functional mobility training; educating patients and caregivers; monitoring progress; documenting visits; and coordinating care with an interdisciplinary team. This is a flexible per diem role with an opportunity for permanent placement. The position includes home visits in the Boynton Beach, Florida area and requires screening through the Care Provider Background Screening Clearinghouse.
Requirements
- Active Florida PTA license.
- Degree from an accredited PTA program.
- Current CPR certification.
- Valid driver’s license and reliable transportation.
- Strong clinical and communication skills.

Responsibilities
