About The Position

The Bridge Family Center is seeking two full-time Family Visitation Coaches for its Mosaic Parenting Center in Cromwell, CT. This role involves case management and supervision of visits between families and children under DCF care, along with all related documentation. While based in Cromwell, significant time will be spent at the Hartford QPC office.

Requirements

  • High School Diploma required.
  • Ability to set professional boundaries and limits.
  • Ability to develop positive rapport with children and adult clients.
  • Ability to address and take control of difficult situations in the moment.
  • Excellent verbal and written communication skills required.
  • Must be able to complete all required documentation in a clear, concise manner consistent with the Visit Coaching model.
  • Must have a valid Connecticut driver’s license.
  • Must possess or be eligible to obtain Adult/Child CPR & First Aid certification after hire.
  • Must possess or be eligible to obtain Therapeutic Crisis Intervention certification after hire.
  • Must possess or be eligible to obtain Visit Coaching Model Training after hire.
  • Must possess or be eligible to obtain additional DCF trainings as determined by the program after hire.

Nice To Haves

  • Associate's or Bachelor's Degree in Human Services, Social Work, Psychology, Education, or a related field preferred.
  • Prior case management and/or visit coaching experience a plus.
  • Bilingual Spanish/English speaking preferred to meet the needs of the clients.

Responsibilities

  • Provide supervision during family visits, maintaining a positive and safe environment.
  • Report any safety concerns immediately.
  • Interact with families during visits, providing guidance on proper engagement and communication with children based on developmental level.
  • Schedule and facilitate initial meetings with biological parents, child, caregiver(s), and DCF upon referral to explain the program, discuss family circumstances, strengths, and needs, and develop or refine the DCF Visitation Plan.
  • Complete an assessment with each family to develop overall goals and meet with the family to develop a visitation plan prior to each visit.
  • Complete and submit all required documentation to all involved parties within established timelines.
  • Communicate families' progress with DCF.
  • Coordinate final discharge meetings with the family and DCF.
  • Provide backup transportation services for visit participants if needed.
  • Perform additional responsibilities as requested.
