Requirements

  • Two years of education in early childhood education, social work, health, psychology, or a related field is recommended (or equivalent degree outside of the US) OR two years supervised work experience with young children and/or parents within a community setting.

  • Pre-existing training in trauma informed care is not a requirement, but a Family Coach must be prepared to utilize the key principles of safety, trust, choice, collaboration, and empowerment.

Responsibilities

  • Recruit and enroll new families in Hired’s Family Home Visiting Program, including through group connections and community outreach.
  • Build and maintain effective relationships with Hired staff and external community partners to support recruitment and referrals.
  • Provide prospective families with information about program services and support successful enrollment and engagement.
  • Build trusting relationships with families and create an environment in which caregivers feel supported, respected, and actively engaged in the home visiting process.
  • Maintain an assigned caseload of up to 20 families and conduct regular visits with families in their homes, community settings, virtually, or in office locations according to program requirements.
  • Maintain a flexible schedule based on family availability and program needs. Family Coaches should expect that their work schedule will not always follow traditional business hours and may include multiple weeknight visits or events per week, occasional weekends, and travel throughout the Twin Cities area.
  • Conduct each visit with fidelity to the PAT model and program expectations, including following required visit components, using curriculum and resources intentionally, and maintaining a consistent standard of quality across families and visits.
  • Come to visits prepared, engaged, and ready to provide families with meaningful support.
  • Maintain an assigned caseload and provide individualized family education and coaching through home visits as well as community-based, virtual, and office visits.
  • Complete family assessments, developmental and health screenings, and track developmental milestones and family and child outcomes according to program requirements.
  • Partner with families to develop, monitor, and review individualized goals.
  • Ensure all required case management components are completed accurately and on schedule, including screenings, assessments, goals, referrals, milestones, and outcomes, and participate in ongoing supervisory review to address follow up needs and maintain program compliance.
  • Maintain foundational knowledge of local community resources related to maternal health, child development, basic needs, and family well-being.
  • Connect families with appropriate community resources and services and support transitions to preschool, kindergarten, and other programs as needed.
  • Provide responsive, strengths based support to families experiencing barriers such as economic instability, housing instability, domestic violence, or other circumstances affecting family well being.
  • Maintain accurate, timely, and complete participant and program records in required electronic case management systems.
  • Document family visits, goals, referrals and resources, screenings and assessments, developmental milestones, and other required program outcomes.
  • Collaborate with supervisor in monitoring documentation and reporting requirements to ensure program standards, contractual obligations, and deadlines are met.
  • Organize and prioritize workload to consistently meet documentation, reporting, and program requirements.
  • Participate in continuous quality improvement activities and use program data and documentation to support service quality.
  • Coordinate event logistics, including reserving and preparing Hired office or community spaces, event setup, facilitation, and cleanup.
  • This includes arranging and transporting catering and necessary event supplies, including food service items and dinnerware.
  • Create opportunities for caregivers to build relationships with one another and strengthen social connections and natural support networks.
  • Facilitate engaging learning opportunities for caregivers related to child development, parenting, family well-being, and other topics relevant to participating families.
  • Market group events internally across Hired programs to increase family participation and generate referrals to the Family Home Visiting Program.
  • Collaborate with other departments and agencies for program activities
  • Meet all program goals as outlined in contract
  • Fulfill obligations as a mandated reporter
