About The Position

Provide case management services for youth and their families with mental health and/or co-occurring disorders as part of the Family Support Team to promote connection and stabilization of family unit. Provide services in the community; including school, home, etc. Attend and participate in team meeting twice weekly. Interact with community partners and other stakeholders, as appropriate. Conduct assessments, create treatment plans, coordinate and provide treatment and supportive services to youth and their families. Provide appropriate referrals for additional services and resources, as identified. Provide support and advocate for children and family’s rights and needs. Assist with identifying non-clinical needs of the youth and families.

Requirements

  • Bachelor’s degree in human services field.
  • Ability to pass a level II Background clearance and drug test.
  • Valid FL drivers’ license, insurance, and safe driving record.
  • Dependable transportation (registered, safe operating conditions, etc.).
