About The Position

World Relief is a global Christian humanitarian organization seeking individuals with compassion and a commitment to justice to join their rapidly expanding team. This organization partners with local churches and communities to address humanitarian crises, strengthen communities, and provide services and advocacy for refugees and immigrants. The Family Support Case Specialist will facilitate group classes for caregivers, connect participants with community resources, support case managers, and provide Medicaid-funded case management services. The ideal candidate will have a proven ability to help immigrants achieve stability and overcome integration barriers.

  • Ability and willingness to drive a 15-passenger van
  • IM+CANS Certified or willingness to become certified
  • Regular travel is required (client’s home and aiding in the connection to other service providers and group sessions)
  • Occasional evenings or weekends required
  • Work and supervise interns
  • Strong Microsoft Office skills required

  • Fluency in Spanish, Arabic, Dari, or Swahili is strongly preferred
  • Strong oral and written English communication ability

Responsibilities

  • Facilitate topic-specific group sessions for caregivers on topics such as self-care, health promotion, psychosocial education, and caregiving resources.
  • Develop curriculum and pre/post tests for group sessions.
  • Work closely with volunteers providing support for group sessions (transportation, childcare, materials, facilitation).
  • Collaborate with the Services to Older Refugee (SOR) team to provide group services for caregivers and older adults.
  • Meet with ethnic community leaders to assess needs and promote group programming.
  • Collaborate and provide training for community partners, mutual aid associations, and churches.
  • Conduct plan of care, assess needs, strengths, barriers, and set goals with each participant receiving individual case management.
  • Provide case management supportive services for individuals funded through Medicaid, including conducting necessary assessments and follow-up through IM+CANS.
  • Exit clients once goals are completed, conducting assessments and closing out the plan of care.
  • Regularly assist other case specialists with case management tasks.
  • Complete other duties as assigned.
  • Write detailed case notes for each task/client interaction, tracking in Ritten (for Medicaid) and Dynamics (for other services).
  • Retrieve participant signatures and complete all necessary program paperwork.
  • Write monthly stories and provide input for grant reports.
  • Research topics and gather information for group sessions and effectiveness utilizing evidence-based practices and peer-reviewed materials.
  • Attend all department, team, and staff meetings.
  • Attend regular off-site Medicaid meetings to discuss cases and mental health topics.
  • Work alongside other staff to conduct seasonal or special events.
