About The Position

Responsibilities include provision of direct services to clients in the Community Support Team ("CST") program through mental health rehabilitative services. This role involves providing direct service to CST clients through mental health rehabilitation services and supports, including illness self-management, skill building, identification and use of adaptive and compensatory skills, identification and use of natural supports and use of community resources. A significant portion of services (60%) must be provided in the member's natural setting. The role also involves gathering member information to assess the need for services, making recommendations for treatment, identifying and investigating existing natural resources and supports, explaining options available to CST clients, and assisting in the application process, all under the direct supervision of a QMHP or LPHA. The Case Manager will work with CST team members to document all service delivery according to Rule 132 and organizational policies. They will also participate in the evaluation of referrals and facilitate placement into the CST program, ensuring clients meet eligibility requirements and all necessary screenings/assessments are completed. This includes coordinating the termination of services, ensuring termination criteria are met, and ensuring an appropriate discharge plan. Through supportive counseling, psycho-education, and case management, the individual will assist people in the program to achieve their treatment goals, maximize independence, and access needed community resources. Coordination with other community agencies and organizations such as DCFS, schools, hospitals, and other providers is also a key responsibility. The role involves providing services in conjunction with other team members to facilitate effective transitions in living arrangements, consistent with the client's welfare and development (e.g., discharge from inpatient psychiatric care, loss of housing). Intensive outreach is required to engage clients in services and those who do not follow through with post-discharge/deflection appointments. Participation in specific program meetings, including mandatory weekly staff meetings and required service line and agency trainings, meetings, and consultations, is also expected. Other duties as assigned.

Requirements

  • Bachelor's degree in psychology, social work or related field required from an accredited college or university.
  • Experience working with clients in a behavioral health setting is required.
  • Bilingual (English/Spanish) required.
  • Must maintain a safe driving record and have current valid Illinois Driver's License.
  • Personal vehicle needed.
