About The Position

MECA Therapies is seeking a Family Service Coordinator (FSC) to join their team. As the largest pediatric rehabilitation provider in New Mexico, MECA Therapies is dedicated to ensuring families and children have access to high-quality care. The FSC will work within the Early Childhood Services department and report to the Regional Director and MECA Management Team. The role involves assisting families of children from birth to three years old in accessing early intervention services, developing the Individualized Family Service Plan (IFSP), coordinating services, and ensuring timely delivery. Services will be provided in home and community settings.

Requirements

  • Bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or a closely related field.
  • Two (2) years of direct experience in serving individuals with disabilities and/or families (if bachelor's degree is in another field).
  • Proficient computer skills in Microsoft Windows.
  • Proficient organizational and record-keeping skills.
  • Effective oral and written communication skills.
  • Excellent interpersonal and teaming skills.
  • Ability to train clients, family members, and professionals.
  • Ability to maintain a high level of confidentiality.
  • Must attend and complete all required service coordination training modules (Part I and II) within one year of the date of hire.

Responsibilities

  • Assist and enable families of children to access and receive early intervention services.
  • Explain the early intervention process, parent rights, and safeguards to families.
  • Explain the ABA process, parent rights, and safeguards to families, as appropriate.
  • Gather information from families regarding their concerns, priorities, and resources.
  • Coordinate evaluation and assessment activities.
  • Facilitate the determination of the child’s eligibility.
  • Connect families with resources and supports.
  • Coordinate with medical and health providers.
  • Help families plan and prepare for their IFSP.
  • Organize and facilitate Transdisciplinary Team Consultation Meetings (TTCM’s) and IFSPs.
  • Arrange and coordinate all services listed on the IFSP.
  • Monitor the delivery of services.
  • Facilitate periodic reviews of the IFSP and ensure a transition plan is developed and transition conferences are held at the appropriate time.
  • Provide families with information and support to enhance their child’s development in cognitive, communication, physical/motor, social or emotional, and adaptive development areas.
  • Support individuals in making choices about their daily lives and finding overall purpose and direction as requested.
  • Complete the Public and Private Insurance form with each family annually to determine eligibility for Medicaid or private insurance coverage.
  • Meet with the family at least once a month through a face-to-face visit.
  • Manage and coordinate the transition process, starting with the transition plan and finalizing at the transition conference.
  • Attend and facilitate monthly TTCM meetings.
  • Ensure the initial visit is face-to-face and at least one face-to-face visit occurs each month.
  • Provide services according to the child’s IFSP.
  • Complete necessary data collection and reporting tools as mandated by MECA executive management, programs, and primary contracting agencies.
  • Create and maintain a confidential record for each person served.
  • Document all time spent with the family and work done on behalf of the family.
  • Enter all documentation into the MAPS system.
  • Complete and submit all monthly documentation and billing submissions in accordance with program-specific guidelines.
  • Complete progress notes with the team at 6-month review TTCM meetings.
  • Provide necessary support, training, and guidance to the persons served.
  • Comply with all mandatory trainings identified by their certification agency and by their supervisor(s) and/or company.
  • Audit a minimum of 3 client files monthly to ensure all documents are accurate and filed appropriately.
  • Acknowledge and respect the rights of persons served at all times.
  • Ensure all service coordination services and related activities are ethical and in compliance with MECA policies and procedures.
