Family Program Clinical Case Manager
About The Position
Marian House is a holistic, healing community supporting women and their children in transitional housing. We provide a safe, structured, and compassionate environment where families can build stability, strengthen recovery, and move toward long-term self-sufficiency. We are seeking a dedicated Family Program Clinical Case Manager to support residents and families in our transitional housing program. This role provides a combination of case management, counseling, and recovery support, helping families achieve stability through individualized planning, community connections, and ongoing therapeutic engagement. You'll play a key role in guiding families through their recovery journey-supporting mental health, housing transitions, and long-term independence.
Requirements
- Master's degree in Social Work, Counseling, Psychology, Family Therapy, or related field
- 3-5 years of counseling and/or case management experience
- Active Maryland licensure (LMSW, LGPC, or equivalent)
- Experience with: Trauma-informed care, Substance use and co-occurring disorders, Evidence-based approaches (e.g., Motivational Interviewing, CBT)
- Strong organizational, documentation, and communication skills
- Ability to build therapeutic relationships and work collaboratively
- Valid driver's license and reliable transportation
Nice To Haves
- In-house clinical supervision for those pursuing LCSW-C or LCPC licensure.
Responsibilities
- Facilitate new resident orientation, including move-in support and development of Individual Recovery Plans (IRPs)
- Provide weekly counseling and case management to families, tracking progress toward goals
- Serve as a therapist, advocate, and care coordinator, connecting residents to: Mental health services, Substance use treatment, Healthcare and community resources
- Support residents in securing permanent housing upon program completion
- Monitor and coordinate drug screening for assigned residents
- Maintain accurate, confidential documentation in compliance with agency standards
- Collaborate with internal teams (program staff, employment, education, and intake)
- Participate in case reviews, staff meetings, and supervision
- Contribute to a team-based environment, including occasional on-call support
