About The Position
The organization is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join its full-time primary care team in Bronx, NY. This is a full-time, outpatient-focused role within a supportive, multidisciplinary clinical environment. The Family Nurse Practitioner will deliver comprehensive primary care services to patients of all ages, with a strong emphasis on preventive care, wellness, and management of acute and chronic conditions.
Requirements
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Active or eligible New York State Family Nurse Practitioner (FNP) license
- National certification required
Nice To Haves
- Prior primary care or family medicine experience preferred
Responsibilities
- Provide comprehensive primary care services, including preventive care, acute illness management, and chronic disease treatment for patients of all ages
- Perform physical examinations; order, interpret, and evaluate diagnostic tests
- Develop and manage individualized treatment plans
- Educate and counsel patients on disease prevention, health maintenance, and lifestyle modification
- Collaborate closely with physicians, specialists, and clinical staff to ensure coordinated, high-quality patient care
- Maintain accurate, timely documentation in the electronic health record (EHR)
- Participate in care coordination, quality improvement initiatives, and population health efforts
