About The Position

Community Wellness Outpatient Mental Health Center, LLC is seeking an experienced Family Nurse Practitioner (FNP) to provide primary and integrated healthcare services across affiliated outpatient behavioral health, substance use treatment, psychiatric rehabilitation, supportive housing, and primary care programs. This is a part-time W-2 role based in Baltimore, MD. The ideal candidate is a licensed Family Nurse Practitioner with experience in primary care, behavioral health, integrated care, addiction medicine, MAT services, clinical documentation, and interdisciplinary care coordination.

Requirements

  • Active, unrestricted Maryland Board of Nursing licensure as a Certified Registered Nurse Practitioner (CRNP).
  • Master’s degree in Nursing from an accredited institution.
  • Board certification as a Family Nurse Practitioner.
  • Valid DEA registration.
  • Maryland CDS registration.
  • Current CPR and First Aid certification.
  • Ability to obtain and maintain professional liability or malpractice insurance.
  • Strong clinical skills in primary care, integrated care, behavioral health, and addiction medicine.
  • Knowledge of COMAR, OHCQ, CARF, SAMHSA, DEA requirements, EHR documentation, ICANotes, and CPT coding.
  • Strong communication, teamwork, clinical judgment, conflict-resolution, crisis response, and decision-making skills.
  • Commitment to trauma-informed, person-centered care is required.

Nice To Haves

  • Minimum of 2–3 years of clinical experience in primary care, behavioral health, or integrated healthcare settings.
  • Experience with Medication-Assisted Treatment services.
  • Experience working within interdisciplinary or integrated care environments.

Responsibilities

  • Conduct medical evaluations, wellness visits, follow-up care, and clinical assessments for individuals enrolled in outpatient behavioral health, substance use treatment, psychiatric rehabilitation, supportive housing, and primary care programs.
  • Diagnose and manage acute and chronic medical conditions, prescribe medications within scope, and provide Medication-Assisted Treatment services, including induction, stabilization, and monitoring when appropriate.
  • Order, review, and interpret diagnostic tests, labs, screenings, medical histories, and treatment plans.
  • Develop individualized care plans, coordinate referrals to specialists, and educate clients and families on wellness, prevention, relapse prevention, medication safety, and treatment expectations.
  • Document services in ICANotes in accordance with COMAR, Medicaid, payer, and organizational standards.
  • Participate in interdisciplinary team meetings, case reviews, staff trainings, quality assurance activities, audit readiness, licensing inspections, and clinical compliance initiatives.
  • Provide consultation for complex cases and respond to after-hours emergencies as assigned.
