About The Position
Community Healthcare Network, a Federally Qualified Health Center (FQHC), serves underserved communities in Brooklyn, Bronx, Manhattan, and Queens, impacting 85,000 lives annually through its Health Centers and programs. This Full-time Family Nurse Practitioner position is based at the South Bronx Health Center. The role involves providing evidence-based Primary Care within a collaborative care team, including nursing staff and services like Health Education, Nutrition, Social Work, and Behavioral Health.
Requirements
- Valid Family Nurse Practitioner N.Y.S. License.
- At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
