About The Position

The Family Nurse Practitioner (FNP) is responsible for providing primary health care services, coordinating and referring clients for health care and health education. This role involves providing comprehensive primary health care, coordinating patient care with other providers and agencies, and advocating for clients within health care systems. The FNP will provide services in prenatal integrated health care of high-risk chemically dependent pregnant patients, in consultation and/or collaboration with an obstetrical health care provider. Responsibilities include performing initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases, including ongoing drug and alcohol assessment and HIV screening. The position also involves encouraging family participation in patient care, developing and documenting plans of care and outcomes evaluations, and coordinating with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births. Health education will be provided to individual patients on topics such as birth control, safer sex, and high-risk behavior and health consequences. Additionally, the FNP will provide staff development education as requested by the Clinic Administrator and engage in ongoing education and specialized training to maintain a high quality of care.

Requirements

Graduation from an accredited Nurse Practitioner program with certification as a Family Nurse Practitioner (FNP) or related primary care specialty.
Nurse Practitioner, unencumbered licensure with State of Oregon as Certified Nurse Practitioner and prescriptive authority.
Current DEA registration or ability to obtain upon hire.
Basic Life Support within introductory period.
Minimum of two (2) years of clinical experience practicing as a Family Nurse Practitioner (FNP), with demonstrated ability to perform the duties outlined above.
Must pass a pre-employment or for cause drug tests.
Must pass criminal background and DMV checks.

Nice To Haves

Experience working within the Native American/Alaska Native community preferred.
Prior experience in primary care, family practice, community health, or federally qualified health center setting preferred.

Responsibilities

Provide health education to individual patients as needed, such as birth control, safer sex, and high risk behavior and health consequences.
Provide staff development education as requested by Clinic Administrator.
Engage in ongoing education and specialized training in order to maintain a high quality of care to Clinic’s patient population.
