About The Position
Community Healthcare Network is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually. We are seeking a Full-Time Family Nurse Practitioner for our South Bronx Health Center located at 81 W. 115th Street New York, NY 10026. Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral part of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
Requirements
- Valid Family Nurse Practitioner N.Y.S. License to deliver medical care.
- At least two (2) year of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
Responsibilities
- Providing evidence-based Primary Care to our patients.
- Being an integral part of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
