About The Position

The Family Nurse Practitioner is responsible for diagnosing and managing acute and chronic conditions and emphasize health promotion and disease prevention. Their services include, but are not limited to ordering, conducting and interpreting diagnostic and laboratory tests; prescribing pharmacologic agents and non-pharmacologic therapies; and teaching and counseling. They practice autonomously and in collaboration with other health care professionals to manage patients’ health needs.

Requirements

  • Graduate of an accredited Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.
  • Must have a permanent and unrestricted license to practice as a nurse practitioner in Arizona.
  • Must have current board certification in family medicine and maintain certification.
  • Must have current medication dispensing license (DEA).
  • At least 3 years of experience in a family practice or comparable setting is required.
  • Current BLS certification is required.
  • Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
  • Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  • Must pass substance abuse testing upon employment and submit to random testing during employment.
  • Must provide an updated immunization (IZ) record at the time of employment.
  • Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.

Nice To Haves

  • Experience in a community health center setting is preferred.
  • Current ACLS certification is preferred.

Responsibilities

  • Provides comprehensive primary patient care including prevention, health maintenance, diagnosis, treatment of acute and chronic conditions, and follow-up services to patients under his/her care within the scope of practice of family medicine.
  • Documents all patient-provider interactions following health care standards.
  • Participates as an active member of an integrated patient centered teaml
  • Supports the organization as needed.
