About The Position

The Community Service Board of Middle Georgia’s is seeking a qualified individual to serve as our next Family Nurse Practitioner (FNP) at our new Behavioral Health Crisis Center (BHCC) in Dublin, GA. This State-of-the-art new facility features 40 total patient beds, including 24 crisis stabilization beds and 16 temporary observation beds, offering both voluntary and involuntary patients access to safe, secure, and compassionate care. This facility will provide critical support and stabilization services for individuals experiencing behavioral health crises in a safe, structured environment where individuals can begin their journey towards healing and recovery.

Requirements

  • Master’s or Doctoral degree in Nursing from an accredited program
  • Current, active, and unrestricted Nurse Practitioner license in the state of practice
  • Board certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
  • Current DEA registration (or ability to obtain prior to start date)
  • Current Basic Life Support (BLS) certification (ACLS preferred, if applicable)

Nice To Haves

  • Minimum of 1–2 years of experience in primary care, internal medicine, urgent care, or similar setting preferred
  • Experience managing acute and chronic medical conditions across the lifespan
  • Experience in behavioral health or working with patients with co-occurring mental health and substance use disorders preferred
  • Experience with medication management and prescribing practices

Responsibilities

  • Conducts medical and psychiatric rounds on CSU/TO individuals at a minimum of every 24 hours. Completes timely and accurate documentation, including initial evaluations, daily progress notes, and discharge summaries.
  • Provides additional documentation as clinically indicated, including behavioral changes, medical concerns requiring evaluation, adverse medication reactions, and other significant changes in condition. Documentation must be individualized and include clear justification for continued stay when applicable.
  • Monitors and manages acute and chronic medical conditions as appropriate within scope, including documentation and follow-up for treatments such as wound care, infections (e.g., UTIs), and other medical needs.
  • Performs psychiatric and medical evaluations for individuals presenting in crisis at the Crisis Stabilization Center (CSC). Initiates appropriate treatment interventions, including medication management and admission orders as needed.
  • Completes medication reconciliation upon admission, ensuring accuracy of home medications. Provides discharge medication reconciliation and ensures prescriptions are sent to the appropriate pharmacy at time of discharge.
  • Reviews and evaluates all referrals for admission. Provides admission orders when appropriate. In cases where admission is declined despite bed availability, participates in and documents a physician-to-physician (Doc-to-Doc) consultation to support clinical decision-making.
  • Actively participates in interdisciplinary treatment team meetings, contributing to care planning, clinical decision-making, and discharge planning.
  • Places and manages all clinical orders, including admission orders, PRN medications, and ongoing treatment orders. Ensures all orders are reviewed, validated, and electronically signed within required timeframes (within 24 hours if applicable).
  • Reviews patient charts regularly to ensure all documentation, orders, and signatures are completed in accordance with organizational and regulatory standards.
  • Provides education to individuals regarding diagnoses, prescribed medications, and aftercare plans at the time of discharge.
  • Completes and signs all required clinical documentation, including legal forms, admission paperwork, treatment plans, and discharge summaries.
  • Assists with clinical assessments as needed, including evaluation of wounds or other medical concerns when requested by staff.
  • Participates in required staff meetings, annual trainings, and competency assessments. May assist in the development and delivery of staff education and training as needed.
  • Supports a safe and therapeutic environment, including assisting with safety procedures such as patient searches when clinically appropriate and in accordance with policy.
  • Maintains compliance with all federal, state, and organizational regulations and standards of care.
  • Practices within the scope of licensure and certification as a Psychiatric Mental Health Nurse Practitioner.
  • Demonstrates strong clinical judgment, crisis intervention skills, and the ability to work in a fast-paced environment.
  • Collaborates effectively with nursing staff, therapists, case managers, and external providers to ensure continuity of care.
