About The Position

Adelante Healthcare is seeking a Family Nurse Practitioner (FNP) Float to provide high-quality, comprehensive primary care to vulnerable populations across 9 clinics in Maricopa County, AZ. The role involves preventive care, health promotion, diagnosis and management of acute and chronic conditions, patient education, and community-focused care. The FNP will work autonomously within a collaborative care team, making a significant impact on community health.

Requirements

• Graduate of an accredited Nurse Practitioner program.
• Active Arizona NP & RN licenses.
• Current DEA certification.
• CPR/AED certification (AHA or Red Cross).
• Strong clinical judgment & patient communication skills.
• Commitment to community health and underserved populations.
• Valid AZ Driver’s License.

Nice To Haves

• 3+ years in community health.
• Bilingual (English/Spanish).

Responsibilities

• Provide high-quality, comprehensive primary care.
• Perform preventive care & health promotion.
• Diagnose, treat & manage acute and chronic conditions.
• Educate patients and provide community-focused care.
• Coordinate care and make referrals as needed.
