About The Position

We are seeking a Board-Certified Family Medicine Physician to join a vibrant outpatient community health practice dedicated to serving an underserved patient population. This facility is associated with a nationally recognized Level I Teaching Hospital and offers a collaborative, patient-centered environment. You'll provide comprehensive primary care services for patients of all ages in a supportive, 100% outpatient setting, ensuring continuity of care while emphasizing preventive health and chronic disease management.

Requirements

  • Board Certification in Family Medicine required.
  • Strong outpatient primary care experience across all age groups.
  • Ability to effectively care for both pediatric and adult patient populations.
  • Excellent diagnostic, clinical, and decision-making skills.
  • Strong communication and patient education abilities.
  • Ability to build trusting relationships with patients and families.
  • Experience collaborating within multidisciplinary healthcare teams.
  • Proficiency with electronic medical records, preferably Epic.
  • Strong organizational, documentation, and time-management skills.
  • Commitment to preventive medicine and patient-centered healthcare delivery.
  • Active DEA certification - REQUIRED
  • Minimum of one (1) year of physician experience required - REQUIRED
  • Active state medical license at the time of submission - REQUIRED
  • Candidates requiring new licensure sponsorship will not be considered - REQUIRED
  • At least one professional reference within the last 24 months - REQUIRED
  • Clean malpractice and license history – HIGHLY PREFERRED

Responsibilities

  • Provide comprehensive outpatient primary care services for pediatric, adolescent, adult, and geriatric patients.
  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Conduct preventive health screenings, routine physical examinations, and wellness visits.
  • Manage chronic diseases and develop individualized treatment plans.
  • Provide pediatric primary care, including well-child visits, immunizations, and management of common pediatric illnesses.
  • Educate patients and families on disease prevention, health maintenance, and treatment plans.
  • Coordinate care with specialists and interdisciplinary healthcare teams.
  • Maintain accurate and compliant documentation within Epic EMR.
  • Participate in limited annual phone/beeper call coverage as assigned.
  • Support quality improvement initiatives and organizational goals.
  • Deliver culturally sensitive and patient-centered care to an underserved community population.
