About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Medicine Physician for NorthShore Health Center in Portage, Indiana. This is a full-time, outpatient primary care position focusing on family medicine. The physician will provide comprehensive care for patients of all ages, diagnosing and managing acute illnesses, chronic diseases, and preventive healthcare needs. The role involves coordinating specialty referrals and continuity of care within a collaborative, team-based primary care model. Dedicated administrative staff will manage operational support, allowing the physician to focus on patient care. The position emphasizes delivering high-quality, relationship-based care in a mission-driven community health setting.

Requirements

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine.
• Eligible for Indiana medical licensure.
• Current or eligible DEA registration.
• Commitment to outpatient primary care.
• Strong communication and collaborative patient care skills.

Responsibilities

• Provide comprehensive outpatient primary care for patients of all ages.
• Diagnose and manage acute illnesses, chronic diseases, and preventive healthcare needs.
• Coordinate specialty referrals and continuity of care.
• Practice within a collaborative, team-based primary care model.
• Focus on patient care while dedicated administrative staff manage operational support.
• Deliver high-quality, relationship-based care in a mission-driven community health setting.

• Base salary of $230,000-$270,000 based on experience.
