We are seeking a dedicated Nurse Practitioner or Physician Assistant to join the Family Medicine team at Atrium Health One Health Mount Island. This position will provide high-quality, comprehensive primary care during extended weekend hours, expanding access and providing greater flexibility for patients and families in the Mount Island community.

Requirements

• Board-certified/board-eligible Nurse Practitioners and Physician Assistants
• Experienced in Family Medicine, Primary Care, or a related outpatient setting.
• Committed to working the Friday, Saturday, and Sunday extended-hours schedule.
• Dedicated to collegiality, professionalism, compassion, personal growth, and leadership development.
• Interested in practicing within a collaborative, provider-led organization that values clinical autonomy and team-based care.

Responsibilities

• Provide comprehensive, patient-centered family medicine care across a broad spectrum of ages, disease states, and chronic conditions.
• Evaluate, diagnose, and treat acute illnesses and injuries within the scope of outpatient family medicine.
• Manage chronic conditions and provide preventive care, health screenings, and patient education.
• Develop individualized treatment plans using evidence-based clinical practices.
• Coordinate care with physicians, specialists, clinical team members, and other healthcare professionals as appropriate.
• Support continuity of care through appropriate follow-up, referrals, and care coordination.
• Utilize One Health and Advocate Health resources, technology, and clinical support services to enhance patient care.
• Contribute to a collaborative practice environment that values clinical expertise, teamwork, and an exceptional patient experience.
