About The Position

Full-time Family Medicine NP or PA needed in McAllen/Edinburg area to provide comprehensive primary care services. This role is part of an integrated healthcare system with six hospitals and offers access to freestanding ERs and a specialty referral network. The position involves supporting physicians in value-based care and HEDIS measures, providing patient support via in-person consults, telemedicine visits, and sponsored health events. The role champions a comprehensive, coordinated model of care for Medicare-aged populations on a Medicare Advantage Plan and Medicaid population. It also involves supporting the attestation process, collaborating with physicians, managed care resources, and regional quality teams. Additionally, the role supports ER and inpatient populations post-discharge to ensure timely follow-up with physicians, preventing readmissions. The position also covers for physicians during absences, rotates on-call duties, educates patients on appropriate use of emergency vs. urgent care, and collaborates with physicians on coding, risk adjustment, documentation, and quality protocols. The NP/PA will interview patients to ensure proper documentation and completion of preventative screenings. Requirements

  • Prior family medicine experience preferred
  • Spanish fluency strongly preferred

Responsibilities

  • Provide comprehensive primary care services.
  • Support physicians to complete Value Based Care and HEDIS Measures.
  • Support patients via in-Person consult, Telemedicine visits, and sponsored health events.
  • Champion a comprehensive, coordinated model of care for Medicare-aged population on a Medicare Advantage Plan and Medicaid population.
  • Champions the Attestation process by collaborating with STHS Clinics physicians, managed care resources, and STHS Clinics Regional Quality.
  • Support our ER and Inpatient population once discharged to ensure STHS Clinics Physicians can see their patients within 7 days to eliminate barriers on discharge and prevent readmissions.
  • Cover for Physicians during vacation time or sick days.
  • Rotate on call after hours and weekends to aid physicians with on call rotation schedules.
  • Educate patients on appropriate use of Emergency Rooms vs. Urgent Cares.
  • Provide Urgent Care Resources.
  • Collaborate with our physicians to educate on proper coding, risk adjustment metrics, capture documentation, and follow quality protocols.
  • Interview patients to ensure proper documentation and that all preventative screenings have been completed.
