About The Position

The Family Health Program (FHP) Coordinator supports the delivery and coordination of comprehensive sexual and reproductive health services within the primary care setting. This role combines program coordination, patient care support, community outreach, staff education, and administrative responsibilities to ensure high-quality, patient-centered care. Working collaboratively with providers, nurses, medical assistants, and administrative staff, the Program Coordinator helps advance the Health Center's mission while promoting equitable access to sexual and reproductive health services.

Requirements

  • Associate's degree in health sciences, public health, human services, or a related field preferred; equivalent education and experience will be considered.
  • Proficiency in written and spoken English.
  • One year of experience in reproductive health, community health, primary care, or a related healthcare setting preferred.
  • Demonstrated interest in reproductive health and justice.
  • Demonstrated ability to work effectively with people of diverse backgrounds, identities, cultures, languages, and lived experiences.
  • Ability to efficiently operate basic office equipment (multi-line phone, fax, photocopier).
  • Ability to use Microsoft Office Suite (Word, Excel, and Outlook) and Internet.
  • Understands and is familiar with basic medical terminology.
  • Excellent interpersonal and communication skills.
  • Strong organizational and time-management skills.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Experience using an electronic health record (Epic preferred).
  • Ability to work independently and collaboratively.
  • Must be able to lift and/or move up to 25 pounds.
  • Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
  • Maintains current CPR certification.

Responsibilities

  • Coordinate the Family Health (FH) Program, including supporting collaboration of medical and reception staff, maintaining and revising forms and materials, managing monthly reporting, assisting in yearly workplan development, and preparing reports and statistics.
  • Support Community Outreach and Health Promotion by providing culturally appropriate health information, coordinating community collaborations with partner organizations, and maintaining relationships with local domestic violence programs and rape crisis centers.
  • Provide Administrative and Patient Support, including guidance to team members, assisting with appointment scheduling, documenting interactions, coordinating care, and monitoring FH task groups.
  • Train and Mentor Staff by participating in new hire orientation, reviewing learner progress, creating a conducive learning environment, demonstrating clinical skills, and assisting with competency assessment.
  • Support Policy and Workflow Development by collaborating to develop, review, and update protocols, establishing workflows, and creating necessary training.
  • Provide Clinical Support, including pre-visit planning, chart review for health screenings, preparing exam rooms, patient intake (vitals, chief complaint, etc.), providing patient education, collecting lab samples, administering CLIA waived tests, acting as a chaperone, preparing providers for procedures, administering vaccines, coordinating in-clinic services, maintaining standard precautions, and assisting in emergencies.
  • Communicate Effectively Across Teams by communicating with patients and staff, sharing pertinent information, maintaining professional communication, and participating in team meetings and quality improvement initiatives.
