About The Position

High Point & Affiliated Organizations is a health and human service agency dedicated to treating and preventing substance use disorders and mental illness. We offer a full continuum of care across Southeastern Massachusetts, including inpatient, outpatient, residential, and community-based services. Our programs also support survivors of abuse, violence, and families experiencing homelessness. We believe in the inherent goodness, worth, and dignity of every individual and aim to help people and families achieve personal change and improve their quality of life. The Plymouth Inpatient campus provides Section 35 services for individuals civilly committed to Substance Use Treatment, including Acute Treatment Services (ATS-detoxification) and Clinical Stabilization Services (CSS). These are staff-secured units, not locked.

Requirements

  • Must present as courteous and professional at all times.
  • Demonstrated flexibility and ability to perform multiple tasks.
  • Excellent verbal and written communication skills, including the ability to organize ideas in a logical and clear fashion.
  • Knowledge of basic computer skills and software programs, including the ability to learn, use, and train High Point & Affiliated Organization's (HPAO) Electronic Heath Records (EHR).
  • Ability to work with others in a cooperative and collaborative manner.
  • Understanding of the relationship between substance use and mental health issues and/or self-help programs.
  • Understand and maintain therapeutic boundaries and reinforce rules in a respectful manner.
  • Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations.
  • TB/Flu and Covid-19 Assessment.
  • Completion of the HPAO approved de-escalation program.
  • Successful completion of all trainings upon hire and annually as required.
  • One-year experience in substance use treatment and/or mental health field.

Nice To Haves

  • Associate's degree, High School diploma, or GED preferred.
  • Associate's degree in human services, preferred.
  • CPR Certification, preferred.
  • Personal lived experience in mental health and/or substance use recovery.
  • Experience working with families impacted by mental health and/or substance use recovery.

Responsibilities

  • Maintains assigned caseload.
  • Provides direct care to clients, including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily group facilitation, group note completion, and completion of bio-psychosocial assessments.
  • Makes referrals when appropriate.
  • Responsible for each assigned patient's medical records, ensuring they are professional, complete, thorough, and updated in a timely manner.
  • Links patients with aftercare services prior to discharge and aids patients and their families in accessing services when appropriate.
  • Strengthens current and creates new partnerships within the community to ensure patients develop skills needed to sustain sobriety and/or housing.
  • Reviews patients/family rights and responsibilities, emergency safety plans, and grievance policy.
  • Communicates and involves family in treatment at the patient's request.
  • Maintains professionalism when connecting with individual's collaterals.
  • Attends and participates in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Director/Program Director/Site Director.
  • Responsible for continued education in the field respective to their service level.
  • Performs other duties as assigned by Clinical Director/Program Director/Site Director/COO.
  • Contacts the insurance company for authorization, as needed.
  • Utilizes walkie-talkie as campus responders.
  • Responds to codes as necessary.
  • Coordinates aftercare services, appointments, and referrals.
  • Facilitates identified groups and milieu activities as assigned.
  • Participates in MDT meetings to support patient care coordination.
  • Assists with monitoring patient observation checks, as needed.
  • Completes required documentation, including: consents/releases, Group Notes, Progress notes, Contact logs.
  • Supports patients in crisis through verbal de-escalation, 1:1 monitoring, or crisis/safety planning, as requested.
  • Attends supervision and staff trainings.
  • Requests extensions after 90 days of treatment if needed.
