The Family Case Manager provides services consistent with the program methodology and standards. This role involves making initial appointments as soon as possible after receiving a referral, meeting with families as dictated by need, and establishing trusting relationships with families at high risk for child maltreatment. The position requires assessing the level of need for case management services, meeting with individuals and families to assess current needs, and ensuring all program criteria are met before opening cases. Outreach with community organizations is necessary to develop relationships for a smooth referral flow. A minimum of one face-to-face contact per month with individuals and families is required, or documentation of reasons for inability to meet. The role includes demonstrating and teaching effective parenting skills, daily living skills such as budgeting and meal preparation, and assessing/monitoring progress with service plan addendums. The Family Case Manager acts as an advocate in staff meetings, IEP meetings, and court, or arranges for a substitute. Linking individuals and families with necessary resources and monitoring their effectiveness is crucial. The position requires contacting individuals/families within 24 hours of a missed appointment and providing culturally competent, solution-focused, strength-based, recovery-oriented, and co-occurring capable case management services. Timely completion of all documentation, including case records, progress notes, and outcome measures, is essential. The role involves using crisis intervention skills, anticipating and preventing crises, staying current on psychotropic medication knowledge, side effects, and DSM diagnostic criteria, and discussing concerns with a supervisor. Consultation with the program manager and assigned clinician(s) regarding cases, reporting all risks to the supervisor, and ensuring risk notes and safety plans are completed are also key responsibilities. Regular meetings with the program manager to review cases and job performance are required. Accurate record-keeping of caseloads and timely completion/reporting of required measures to data entry are necessary. Meeting required productivity standards, assisting with quarterly and annual reports, and accomplishing individual goals are also part of the job.
