About The Position

Merakey is seeking a Family Visitation Case Manager to join their Children's Program serving Clients in Sussex County, Delaware. This role involves coordinating resources and information pertinent to client needs, documenting all coordination efforts, and managing referrals for client-specific discharge planning. The Case Manager will ensure family participation in creating a discharge plan that reflects their choices for service providers and community resources. Responsibilities also include monitoring record keeping, compliance, and client chart updates, including incident reports within 24 hours. The role requires monitoring and documenting safety, and completing case documentation in compliance with standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations. A key aspect of the role is supervising family visits using the Marty Byer philosophy.

Requirements

• Bachelor's degree in psychology, sociology or social work
• At least 1 year of experience working with children/adolescents
• Valid Driver's License and clean driving record

Responsibilities

• Coordinates resources and information pertinent to client needs.
• Effectively and efficiently documents all coordination of resources.
• Coordinates referrals for client specific discharge planning; ensures that the family actively participates in creating a multi-faceted discharge plan; one which represents the family’s choice for service providers, and community resources.
• Monitors record keeping, compliance and authorized units remaining, ensures that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
• Monitors/assesses and documents safety
• Completes necessary documentation of each case, which complies with the standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.
• Supervise family visits using the Marty Byer philosophy
