About The Position

The Maternal Health Supervisor is a Family Case Manager who is responsible for coordinating enrollment with a team of Home Visitors to provide services to mothers within the Maternal Health Home Based Services Program. The role coordinates and or conducts well baby check-ups and screenings of both the child and the mother during the first 18 months after the child is born. Tasks include: develop and implement program using evidence based tools, coordinate enrollment, hire, train and manage direct reports, distribute and monitor caseloads, develop action plans to monitor and report progress, help referred families find community resources such as healthcare, mental health services, housing, financial assistance, employment, transportation, childcare, etc. Home visits are conducted to identify parenting needs and to provide support for the family and the child’s development and well-being. Encouraging a healthy and nurturing lifestyle, and assisting with plans, considering education and assistance finding work, will be provided as well.

Requirements

  • Must be at least 21 years of age.
  • Bachelor’s Degree in Human Services or related field.
  • Two (2) years’ working in a Human Services profession.
  • Excellent communication skills (verbal and written) with the ability to articulate information to a diverse audience.
  • Proficient with Microsoft Office Suite.
  • Current driver’s license and the ability to travel for business utilizing personal vehicle.

  • One (1) year of Supervisory experience.
